5-year work history evaluation
Verification of a current state license that is in good standing
An NPDB (National Practitioner Data Bank) report, which will tell us if a provider has any malpractice claims history or state licensure actions
A current DEA or CDS (This allows the dentist to write prescriptions)
Current malpractice insurance must be maintained
Inquiry for Medicare/Medicaid Fraud or opting out of Medicare
This dentist vetting process typically takes 7-10 business days but could take longer in some cases
